Required Information for Card Design and Layout

• Design layout of both front and back side of card
• All logo and graphic image files in acceptable format and/or hard copy sample
• All text placement and applicable font types, sizes and colours
• Bar code placement

Required Information for Bar Coding and Numbering

• Sample of previous bar code, if available
• Bar code symbology—ie. Code 3 of 9, Code bar, Code 128, Interleaved 2 of 5 (Follett) OR none, if only numbering required.
• Start sequence number and ending sequence number
• Total number of digits in bar code
• Format and positioning of eye-readable number
• Check digit requirements, if any
